Accessing the Patient Portal:

The San Ysidro Health Patient Portal lets you view your health information online and electronically communicate with your provider and care team. By using the patient portal, you can also:

  • Access and request personal or general information related to your care
  • See how much money you owe for your care and make payments online
  • Access educational materials on topics that are related to your health
  • Request medication refills
  • Receive messages or documents from your doctor or care team. These may include lab results, radiology results, referrals, letters, and patient plans

You can also sign up for the Patient Portal during your next visit to a San Ysidro Health clinic location. Talk to any Patient Services Representative (located at your clinic’s registration desk) for help with registration.  If you are having difficulty registering, click here to access helpful tips and FAQs.

Mobile Access:

You can access the San Ysidro Health Patient portal using your smartphone or tablet. Download the NextGen Patient Portal Healthcare App to conveniently access your health information from anywhere.

Privacy:

Privacy is a priority at San Ysidro Health. San Ysidro Health’s Patient Portal is a secure place that gives registered users access to online health care information. This system is paperless and patient information will be accessible via the portal. If a paper document is required or you wish to delete your patient portal account, please contact (619) 662-4100.

No health information will be sent via email. When a message is sent from your provider, you will receive an email stating that you have a new message from San Ysidro Health and are directed to log into the San Ysidro Health Patient Portal to view the message.

If access is requested for any dependents/care managers to your portal account, it is required that you and the requested dependents/care managers are present at a San Ysidro Health clinic location at the time of enrollment. This is done to protect the privacy of our patients.

Enrollment for patients ages 0 to 17 and conservators of adult patients must also be done in-person at one of our clinic locations.
