How to Sign Up for MySanYsidroHealth
Signing Up With an Activation Code
Go to mychart.syhealth.org on your phone or on your computer. These screens will look similar on either device. If you would like to view the page in Spanish, click Ver en Español.
Step 1
From the login screen, select Sign Up Now.
Step 2
Enter your activation code and other personal information, such as zip code and your date of birth. Select Next.
Step 3
On the next page, choose the following:
MySanYsidroHealth username: Choose something that others wouldn't be likely to guess but is easy for you to remember. It cannot be changed at any time.
Password: Choose a unique combination of numbers and letters, using both uppercase and lowercase letters. Your password must be different from your MySanYsidroHealth username. Choose a password that you don't use for other websites.
You will accept the Terms and Conditions after creating a username and password.
Step 4
On the final screen, choose whether you want to receive notification messages in your personal email or mobile phone number when there is new information available in your MySanYsidroHealth account. If you opt to receive email alerts, you will enter your email address. If you opt to receive text updates, you will enter your mobile phone number.
Signing Up Without an Activation Code
Go to mychart.syhealth.org on your phone or on your computer. These screens will look similar on either device. If you would like to view the page in Spanish, click Ver en Español.
Step 1
On the login screen, click Sign Up Now.
Step 2
Once on the Sign Up Now page, look for the No Activation Code? prompt and click Sign Up Online.
Step 3
Enter your personal information on the Sign up for MySanYsidroHealth page. Follow these steps to sign up for a MySanYsidroHealth account. Complete the CAPTCHA and click Next.
Step 4
Click Verify Now on the next page.
Step 5
Verify your identity through Experian Precise ID and provide personal information, such as your date of birth, and have an activation code sent to your email address or mobile phone.
Note: Experian Precise ID is a third-party verification system that confirms your identity by checking responses that you provide against their database of information. None of this information is stored at your clinic or by San Ysidro Health.
Step 6
After verifying your identity, you may be prompted to set up Two-Step Verification. You can either click Continue or turn off two-step verification.
Step 7
If you press Continue, you will be taken to another page to input your email address for two-step verification. You will receive a code via email after pressing Continue again.
Step 8
After going through the verification steps and entering your code, you will choose a username and password. Follow the directions on this screen closely, so that your username and password meet the site requirements. Click Next when you are done.
Step 9
On the next screen, you will set your notification preferences for your account. Follow the prompts, then click Sign in.
Step 10
As a final step, you will be asked to accept the Terms and Conditions of Use for MySanYsidroHealth. Press Accept to finish your account set-up.
