Adult One-on-One Health Education (18+ years of age)

San Ysidro Health offers free Health Education sessions to help patients maintain a healthy lifestyle, while they learn how to manage their health conditions, including weight management, obesity, hypertension, pre-diabetes, and type 2 diabetes.

The sessions are delivered by our Health Educators, who design each set of sessions around the individual needs of the patient.  The Health Educator works with the patient to set attainable and meaningful self-management goals in order to build healthy lifestyle habits into their daily routine, prevent complications, and enhance their quality of life.  The Health Educator will also help patients address specific barriers they are facing and provide resources to help with the changes they want to make.

Sessions are primarily delivered over the phone, with some in-person appointments available.  The outline of the curriculum is shown below, however the Health Educator will adapt this to meet the individual needs of each patient:
