Sessions are held individually with the patient, a family member, and our Health Educator. Sessions are primarily delivered over the phone, with some in-person sessions available. The outline of the curriculum is shown below, however the Health Educator will adapt to meet the individual needs of each patient and help them to set achievable goals. Through this approach, small lifestyle changes can lead to large health benefits.
