San Diego PACE’s state-of-the-art facilities proudly serve 114 zip codes throughout San Diego. We encompass and provide life-changing medical care to the County’s South, Central, East, and North regions.
San Ysidro was our first center to open its doors in 2015, and it is conveniently located off the 905 freeway, which provides easy access from the 5 and 805 freeways for the South Bay senior community.

Our Chula Vista center is our second center that opened in 2019, and it is conveniently located on the corner of L Street and 3rd Avenue with access from the 5 and 805 freeways.
El Cajon is our third facility in operation. Opened in 2021, this modern, state-of-the-art site is a two-story building that also houses a San Ysidro Health clinic on the 2nd floor. It is located near the El Cajon Boulevard exit off the 8 freeway providing easy access to our senior population in East County.
In 2022, we opened our fourth center location and our first in North County. This two-story building in Vista is conveniently located off the Emerald Drive exit from the 78 highway. This site is near the Oceanside and Vista area and serves the Northern San Diego region, including Fallbrook, Bonsall, and Valley Center.
