Seniors who attend the day center also have an opportunity to participate in recreational activities, eat healthy meals, and develop new friendships and companionship. In addition, trained and specialized staff provide nutritional counseling, physical therapy, occupational therapy, exercise, and more. Our goal is to offer comprehensive care and activities that help you stay healthy and active and develop a strong sense of community.

Behavioral Health

Recognizing the multiple needs in the communities we serve, San Diego PACE has moved towards a fully integrated care model in order to provide supportive services beyond the scope of traditional primary care. Our integrated care model allows both primary care and behavioral health to work together.

The San Diego PACE Behavioral Health team works closely with the PCP (Primary Care Provider) to address a variety of concerns including depression, anxiety, sleep, substance use, etc.
